NeuHub may collect two types of information:

Includes health-related data collected as part of your participation in NeuHub cognitive programs. PHI may include:

  • Cognitive assessments and progress reports
  • Session notes from coaching or facilitation
  • Goals, journaling, or other health-related information entered into our HIPAA-compliant Electronic Health Record (EHR) system

  • Enroll you in cognitive health programs
  • Provide coaching, facilitation, and support services
  • Document your progress and goals securely in our EHR

  • Process billing and payments
  • Comply with legal and regulatory obligations

5. Sharing of Information

NeuHub may share your PHI only as permitted under HIPAA:

  • With your explicit written authorization
  • For treatmentpayment, or healthcare operations
  • With business associates who help us deliver services and are bound by a Business Associate Agreement (BAA)
  • As required by law, including for public health or law enforcement purposes

We do not sell or use your PHI for marketing without your explicit consent.


6. Cookies & Analytics

Non-personal information (e.g., browser type, page visits) may be collected through cookies for analytics and site improvement purposes. This does not include PHI.

We may update this Privacy Policy from time to time. All updates will be posted on this page with a revised effective date.
